NO PURCHASE OR DONATION NECESSARY TO ENTER OR WIN. A DONATION OR PAYMENT WILL NOT INCREASE YOUR CHANCES OF WINNING.

1. Sponsor. Thrive Life Project ("Sponsor"). Website: thrivelifeproject.org. Mailing Address: [Add address].

2. Eligibility. Open only to legal residents of Utah, Idaho, Montana, Nevada, and Wyoming who are 18 years of age or older at time of entry. Void where prohibited. Employees, officers, directors, contractors, and volunteers of Sponsor and members of their immediate families/households are not eligible.

3. Promotion Period. Begins September 1, 2025 at 12:00 a.m. Mountain Time and ends September 30, 2025 at 11:59 p.m. Mountain Time (the “Promotion Period”).

4. How to Enter. During the Promotion Period:

a) Free Method of Entry (AMOE): Submit the online form on this page (name, email, state of residence). Limit one (1) AMOE submission per person per calendar day.

b) Donation Entry (Optional): Donate to Thrive Life Project using the Donate link on this page. For each $35 donated in a single transaction you will receive one (1) entry (e.g., $70 = 2 entries; $350 = 10 entries). No entry limit. Donations are non-refundable. Entries from donations and AMOE have equal chance of winning.

Entry Conditions: Automated or bulk entries are prohibited. Incomplete, illegible, or fraudulent entries are void. Sponsor’s database clock is the official timekeeping device.

5. Prize; ARV; Odds. One (1) prize package consisting of a Nintendo Switch 2 and a Samsung 65” 4K TV (total ARV $2,000). Model and features determined by Sponsor in its sole discretion. Odds of winning depend on the number of eligible entries received. No cash alternative except as expressly provided in Section 6.

6. Prize Delivery & Substitution. Prize will be shipped via UPS or FedEx to a valid U.S. address within UT, ID, MT, NV, or WY. If shipment is not reasonably possible (as determined by Sponsor), winner must elect gift card(s) equal to ARV or arrange in‑person pickup in Vernal, Utah within 30 days of notice. Sponsor may substitute a prize of equal or greater value if the advertised prize becomes unavailable. Winner is solely responsible for any taxes and fees associated with prize receipt/use. Sponsor will issue IRS Form 1099 if required by law.

7. Winner Selection & Notification. One (1) potential winner will be selected in a random drawing on or about October 5, 2025 from among all eligible entries received. Potential winner will be notified via the email provided (and/or direct message if entered via social) and must respond within 7 days with eligibility confirmation; otherwise, an alternate winner may be selected.

8. Publicity & Privacy. By accepting the prize, winner grants Sponsor the right to use winner’s name, city/state, likeness, and statements for advertising and promotional purposes in any media without further compensation, unless prohibited by law. Personal data will be handled per Sponsor’s Privacy Policy.

9. General Conditions. Sponsor reserves the right to disqualify any individual who tampers with the entry process or violates the Rules, and to cancel, suspend, or modify the Promotion if fraud, technical failures, or any other factor impairs the integrity of the Promotion. Proof of submission does not constitute proof of receipt. Released Parties (Meta, Nintendo, Samsung, UPS, FedEx) are not sponsors of this Promotion and are released from all liability.

10. Limitations of Liability. By entering, entrants agree to release and hold harmless Sponsor and its respective officers, directors, employees, and agents from any liability for any injuries, loss, or damage of any kind arising from or in connection with participation or the acceptance, use, or misuse of the prize.

11. Disputes. Except where prohibited, entrants agree that all disputes will be resolved individually, without resort to class action, and exclusively by the state or federal courts located in Utah, and governed by Utah law.
